For example, a client may speak of the experience of a friend who became worse in therapy rather than better.

Thus we find in the therapeutic situation repetitions of emotional relationships that the client had experienced in childhood—transference—and hindrances to the undoing of repression—resistance. Conclusion Psychoanalytic treatment addresses those manifestations of personality that can be defined as neurotic, that is, characterized by symptoms (compromise formations that are incongruent with the individual's conscious intentions) and by suffering, because the symptoms lead to behavior that is maladaptive.

If, however, the person believes the symptomatic behavior seems acceptable, or at any rate seems not to be causing so much suffering that he or she would risk giving up this behavior, these symptoms can be called ego-syntonic. Ego-syntonic symptoms may consist of character traits and behavioral dispositions, of the typical ways in which a person relates to other persons; therefore such symptomatic behavior may be less obvious than psychosomatic reactions, depression, or anxiety. The difficulties of persons whose symptoms are ego-syntonic are frequently mostly characterological; and so one speaks of the disorder that these persons exhibit as character neurosis.
